* This is the primary vehicle to be driven by this student.

1.) All student automobiles parked on school grounds must be registered with the school and display the current school year parking permit tag.
a) Parking tag must be displayed facing outward hung on the inside of the rearview mirror.
2.)A fee of five dollars is charged for the parking permit tag.
3.)Any change in the vehicle being driven must be reported ot the office and new or additional registration completed.
a.) If the vehicle is sold the registration for that vehicle must be cancelled.
b.) If a different vehicle is obtained the registration must be changed to it.
c.) If an additional vehicle is to be used it must be added to a student's registration.
4.) Student parking on school grounds is considered a privilege. Parking regulations are strictly enforced. Suspension of parking privileges, towing of vehicle and/or other disciplinary action may occur for parking violations.
5.) Student parking is strictly limited to the student parking area.
6.)The Portage County Sheriff will handle violations in the handicapped parking areas and Fire Lanes.
7.)Student drivers must be licensed.
a.)Student driver must present a valid Ohio Driver's License at time of parking registration.
b.)Any suspension or revulsion of a student's Driver's License must be reported to the High School Assistant Principal.
8.) Student must be covered by insurance for the vehicle being driven.
a.) A photocopy of the Ohio Proof of Insurance form must accompany the student parking registration form.
b.)A photocopy of the Ohio Proof of Insurance form is required for each vehicle being registered.
9.) Parking on school grounds is limited to seniors, juniors, and sophomores.
a.)Freshman may not park on school grounds.
10.) Students will not loiter in the parking area, nor may they sit in their vehicle while parked on school grounds.
a.)Students must enter the building upon arrival at school.
11.)Students must park vehicle upon arriving at school.
a.)Student may not "cruise" the parking lot or other areas of the school district.
12.)Vehicle will be appropriately parked.
a.) Pull in parking only will be permitted.
b.) Parking lot speed shall not exceed 5MPH
c.) Reckless or irresponsible operation of the vehicle is prohibited
13.) Student vehicles are subject to search.
14.) All rules in the Student Conduct Code apply to the parking lot.
15.) At the end of the school day students exiting the parking lot to travel West on Tallmadge Road must use the West exit of the student parking lot onto Tallmadge Road.
a.) Once the busses have begun to exit, students must wait until all the busses have departed before they may depart the parking lot.
b.) Students may not use the drive along the front of the building to exit the parking lot.
16.) At the end of the school day, students exiting the parking lot to travel East on Tallmadge Road must use the East exit, near the football field, to leave the student parking lot.
17.) Students may only drive vehicles owned by their parent, guardian, or themselves.

I understand and I am aware of the conditions contained within this contract, I also understand the consequences should I violate any of the provisions of this contract.
