Enrollment Requirements

The enrollment procedure is considered finished when the following gorms have been completed and the necessary conditions have been fulfilled:

(1) Enrollment form (including authorized parent/gaurdian signature)

(2) Request for Records form

(3) Emergency Medical Forms (after school begins)

Upon Enrollment the state of ohio requires the parent/gaurdian provide:

• Birth certificate
• Custody papers (if applicable)
• Social Security Card (Copied)
• Proof of Residency
• Last report or unofficial transcript
• health records
• Special Education--IEP (if avalible)
• Photo ID of person enrolling

Note: A student may not enter if they are under a Suspension or Expulsion from another District.
